License renewal is every even-numbered year on the following dates:

• DOs must renew by November 30th.
• MDs must renew by December 31st.

Fees

How much does it cost to renew in 2026?

The Pennsylvania renewal fees are as follows:

• DOs $450 (see 49 Pa. Code § 25.231).
• MDs $360 (see 49 Pa. Code § 16.15 and 49 Pa. Code § 16.13).

These fees apply to both an active and active-retired license.

If you have a compact license, you will need to pay the Pennsylvania license renewal fees plus $25 for the IMLC renewal fee. Compact licensees will pay the Pennsylvania and the IMLC renewal fees through the IMLC website, https://imlcc.com/renew. 

How much does it cost to renew a volunteer license?

There is no fee to renew a volunteer license.
