Comply with ID Badge Requirements with Template

Specific provisions of the 2011 Photo Identification Tag law went into effect on June 1, 2015. This law is applicable to employees and physicians working in private practices, health care facilities licensed by DOH, and employment agencies.

To print your own temporary badges, use the Pennsylvania Medical Society's (PAMED's) template as outlined below.

To print your ID badge, follow these steps:

  1. Download the Template. The template, which also contains a sample badge, should open in Microsoft Word.
  2. Type in your first name, last name, title, and facility name where indicated on the template. Add your photo to the badge. From Microsoft Word, go to Insert Picture, and locate the photo file on your computer:
    • Medical and Osteopathic Doctors must use the title “Physician.”
    • Registered Nurses must use the title “Registered Nurse.”
    • Licensed Practical Nurses must use the title “Licensed Practical Nurse.” 

    The law does not specify:

    • The title to be used for employees who provide direct patient care, are licensed by the state, but are not licensed in one of the aforementioned professions (e.g., physician assistants).
    • The title to be used for employees who provide direct patient care and are not licensed by the state (e.g., medical assistants).
    • Badge size
    • Photo size or whether the photo is in color or black and white
    • Font type or size
    • With the exception of the title, location on the badge for any of the required information.
  3. Print your badges. This ID badge template is compatible with Avery products 5390, 5383, 74461, 74549, and 74556. Most Avery products can be purchased at office supply stores, such as Office Max or Staples.
