    After reviewing the Informed Consent Quick Consult item I have the following questions:

    1. Should there be a signed consent by the patient and physician for each treatment intervention, e.g. new prescription, recommended and provided to the patient or is a generic and global Informed Consent signed by the patient at the time of entry into a practice suffice for activities within the scope of a practice?

    2. Should there be a signed consent by the patient and physician for each off-label use of medication with the rationale and risks for this recommendation or again is this covered by the generic admission Informed Consent form?
