Chronic Care Management – Incorporating Social Drivers of Health Project

The Pennsylvania Medical Society (PAMED) is offering a new Chronic Care Management – Incorporating Social
Drivers of Health as a value-added program for members. The purpose of the project is to engage physician practices in opportunities to support the incorporation of social drivers of health screenings and tools into chronic disease management. This
is a year-long project beginning January 2, 2023.
PAMED will select up to 20 practices in underserved communities for the project. The practice will choose one chronic condition and identify 1-2 quality improvement metrics to target throughout the year, along with corresponding social drivers of health.
Physicians will receive 20+ Category 1 CME credits and participating practices will receive an administrative stipend to get started.
Rules for applying:
- Must be a member of PAMED
- Must be a physician-focused project. For every 5 participating
physicians, one CRNP or PA may be included.
- Each project will target only ONE chronic condition in the practice.
