The documents below are the most commonly used CME document and are required for accreditation through the Pennsylvania Medical Society. Documents require either Microsoft Word or Microsoft Excel.

Online CME Disclosure of Financial Relationships Form
As a Category 1 CME provider, accredited by the ACCME, the Pennsylvania Medical Society requires that all individuals involved in the planning and execution of a Category 1 continuing medical education activity, as either content developers or faculty, provide disclosure on any financial relationships that they may have or that their immediate family may have with commercial interests.
Disclosure Form (Word)
Anyone involved in the planning and execution of a Category 1 CME activity must use this form to disclose all relevant financial relationships.
Speaker Presentation Agreement for a Live Activity (Word)
This form conveys educational objectives, disclosure requirements, and ACCME commercial support standards to presenters.
Faculty Presentation Agreement for Enduring Material (Word)
This form conveys educational objectives, disclosure requirements, and ACCME commercial support standards to faculty or authors.
Conflict of Interest Reporting Form (Word)
Conflicts of interest for anyone involved in the content of a CME activity must be listed on this form. Use this form to report how conflicts of interest were handled or that there were no conflicts of interest.
Content Review Questionnaire (Word)
This form can be used to manage potential conflicts of interest.
Financial Reporting Form (Excel)
Use this spreadsheet to report required financial information for your activity.
Activity Evaluation Form (Word)
Use this evaluation form to get feedback from participants and to assess change in your learners.
Commercial Support Letter of Agreement (Word)
This letter must be completed if a CME activity receives commercial support. (Requires Microsoft Word)
